🎯 What You'll Do

• Conduct direct outreach to patients for enrollment and engagement
• Facilitate virtual appointments with providers
• Ensure patient adherence to care plans
• Coordinate care across interdisciplinary team

📋 Requirements

• 2+ years of relevant work experience (e.g., Community Health Worker, Medical Assistant)
• High school diploma or GED required
• Valid driver’s license, auto liability insurance, and reliable transportation
• Experience in chronic care management or with chronically ill/elderly patients
