💼 About This Role

You'll provide home health visits to adult and geriatric patients in Plymouth County, following care plans and administering treatments. You'll monitor client conditions and coordinate care with the clinical team while enjoying a consistent evening schedule starting at 3pm.

🎯 What You'll Do

• Follow designated care plans for patients
• Make home visits to patients in assigned areas
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to clinical manager
• Document observations and interventions using a tablet

📋 Requirements

• Current Registered Nurse license in Massachusetts
• Graduation from an accredited nursing program
• Medicare Certified Home Health Visits experience required
