💼 About This Role

You'll serve as attending physician for patients in skilled nursing facilities, collaborating with NPs/PAs to manage daily care. You'll provide high-quality primary care and participate in quality initiatives like Chronic Care Management. This role offers flexible scheduling, autonomy, and productivity-based compensation with bonuses up to $50,000.

🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
• Provide high-quality, patient-centered primary care for acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, and Remote Patient Monitoring.

📋 Requirements

• Active U.S. medical license and board certification or board eligibility in appropriate specialty.
• MD/DO degree.
• Ability to perform essential job functions with or without reasonable accommodation.
