Description

You will provide home health speech therapy services to adult and geriatric patients across multiple counties. As part of a multi-disciplinary team, you'll perform evaluations, develop treatment plans, and educate patients and families, using a touch pad tablet for documentation.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational requirements
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Perform home visits and thorough evaluations for speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluations, physician orders, and goals
• Continually assess and revise care plans; participate in multidisciplinary care planning
• Educate and instruct patients and families on rehabilitative care and activities
• Document observations, interventions, and evaluations using an electronic tablet
