Description

You will be a Lead Case Manager, outreaching and engaging individuals with serious mental illness, substance use disorder, homelessness, or high medical needs to connect them with whole-person care. This role focuses on building trust and relationships to serve underserved populations, primarily remote with 2-3 on-site visits per month.

Requirements

• Bilingual in English and Spanish
• Experience working with underserved populations
• Strong communication and relationship-building skills
• Ability to work remotely with occasional on-site visits
• Passion for mission-driven healthcare

Responsibilities

• Outreach and engage individuals with SMI/SUD, homelessness, or high medical needs
• Build trusting relationships to facilitate whole-person care
• Coordinate care across clinical, behavioral, and social domains
• Support patients in accessing timely, personalized support
• Collaborate with interdisciplinary care team
