Description

As an RN Rehab Nurse, you will assess residents' health status, develop dynamic care plans, and collaborate with an interdisciplinary team to ensure high-quality care. You will perform nursing interventions such as medication and IV administration, communicate changes to physicians using SBARR, and participate in quality improvement processes.

Requirements

• Current RN license in the state
• CPR/AED certification or willing to obtain within six months
• Long-term care experience preferred
• Ability to stand/walk for long periods and lift heavy objects
• Excellent communication and critical thinking skills

Responsibilities

• Assess residents' health status on admission and ongoing
• Develop and revise interdisciplinary care plans based on residents' needs
• Administer medications, treatments, and IV fluids safely and timely
• Communicate significant changes to physicians using SBARR
• Monitor care delivery and address problems using QAPI process
