Description

You will perform home health visits for adult and geriatric patients in Frederick County, MD. As part of a multi-disciplinary team, you will provide rehabilitative care and skilled nursing to help clients remain at home.

Requirements

• Current Speech Language Pathologist license in MD
• Certificate of Clinical Competence in Speech from ASHA or equivalent
• Solid computer skills; prior EMR experience preferred
• Prior home care experience is a plus

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations for speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation and physician's orders
• Educate patients and family members on rehabilitative care
• Document observations and interventions using electronic tablet
