Description

You will provide skilled professional nursing care to hospice patients and their families, managing a primary caseload and collaborating with the interdisciplinary team to ensure compassionate, person-centered care. Your work will support a mission-focused ministry transforming elder care delivery.

Requirements

• Graduate of approved nursing program
• Valid Ohio Registered Nurse (RN) license
• Minimum 2-4 years direct patient care experience in hospice or home care
• Bachelor of Science in Nursing (BSN) preferred

Responsibilities

• Manage primary caseload of hospice patients
• Provide skilled professional nursing care as prescribed by physician
• Comply with Hospice Medicare Conditions of Participation and Ohio licensure laws
• Support patients, families, and caregivers in end-of-life care
