Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them maintain independence. You'll work as part of a multidisciplinary team, developing and implementing individualized care plans.

Requirements

• Current NJ Physical Therapist license.
• One year prior clinical experience as a licensed Physical Therapist preferred.
• Experience with adult and geriatric populations.
• Ability to perform home health visits in Cape May County.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.).
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care and activities for independence.
• Document observations and interventions using a touch pad tablet.
