💼 About This Role

You'll provide home health visits to seniors in Independent and Assisted Living communities in New Castle County. You'll develop and implement individualized care plans to promote independence and safety. This role offers career advancement and recognition programs.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility and strength.
• Develop and implement individualized physical therapy care plans.
• Educate clients and family members on rehabilitative care.
• Document client care using electronic touch pad tablet.

📋 Requirements

• Current DE Physical Therapist license.
• Minimum one year of recent experience as a Physical Therapist.
• Graduation from an APTA-approved program.
