💼 About This Role

You'll join a multidisciplinary team visiting clients in their homes to assess social and emotional factors, plan interventions, and connect them with community resources. Your work enables patients to remain at home while receiving needed medical care. You'll be supported with PPE and comprehensive infection prevention protocols.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify community resource referrals for clients and families.
• Document observations and interventions using a touch pad tablet.

📋 Requirements

• Licensed by VA Board of Social Work Examiners at LMSW or LISW level.
• Master's in Social Work from a CSWE-accredited program.
• At least 2 years of community-based experience.
