Description

You'll provide rehabilitative care and skilled nursing to clients in their homes, conducting evaluations and developing treatment plans for speech, language, communication, or swallowing difficulties. You'll work as part of a multi-disciplinary team to help clients remain at home.

Requirements

• Current Speech-Language Pathologist license in Massachusetts
• One year experience with adult/geriatric population

Responsibilities

• Conduct in-home evaluations to assess speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluations, physician orders, and goals
• Continuously assess and revise speech therapy care plans
• Educate patients and families on rehabilitative care and activities
• Document patient care management and services using a touch pad tablet
