You'll lead a team of RN Case Managers and LVNs, ensuring coordinated patient care in a rapidly growing home health agency. Your core impact includes reducing hospital readmissions through supervisory visits and educational programs.

🎯 What You'll Do

• Coordinate patient care oversight with RN Case Managers and LVNs
• Provide timely support as first point of escalation
• Develop and implement nursing educational programs
• Conduct bi-weekly case conferences to review patient progress

📋 Requirements

• Two years management or supervisory experience in healthcare
• CA Registered Nurse License (current)
• Basic Life Support (BLS) Card (current)
• Valid CA Driver's License with acceptable driving record
