💼 About This Role

You'll conduct in-home health assessments for patients across Michigan, supporting Porter's healthcare transparency mission. Travel accommodations are covered, including hotels and rental car reimbursement.

🎯 What You'll Do

• Perform in-home health risk assessments including medical history and physical exam
• Conduct medication reconciliation and point-of-care testing
• Identify diagnoses for care management and treatment planning

📋 Requirements

• Valid unencumbered Michigan NP license
• 3 years of patient care experience
• Proficiency with EMR systems
