Description

You will provide occupational therapy to adult and geriatric patients in their homes, as part of a multidisciplinary team. Responsibilities include evaluating functional status, developing individualized care plans, and educating patients and families to promote independence and safety.

Requirements

• Current STATE Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status.
• Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans.
• Evaluate home environment for hazards or barriers and identify equipment needs.
• Educate and instruct patients, family members, or other patient representatives in rehabilitative care.
