💼 About This Role

You'll complete OASIS Starts of Care for adult and geriatric patients in Essex County as part of a multi-disciplinary home health team. You will make home visits, perform diagnostic tests, and develop individualized care plans. This role offers a $10,000 sign-on bonus and comprehensive benefits.

🎯 What You'll Do

• Conduct Start of Care visits and OASIS assessments
• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Educate clients and families on rehabilitative care

📋 Requirements

• Current New Jersey Physical Therapist License
• Minimum one year of recent PT experience
• OASIS experience is required
• Home health experience required
