Description

Join our home health team to provide speech therapy services to adult and geriatric patients in their homes. You'll conduct evaluations, develop treatment plans, and collaborate with a multi-disciplinary team to help clients remain independent.

Requirements

• Current Vermont Speech Language Pathologist license
• Certificate of Clinical Competence in Speech (CCC-SLP) or equivalent
• Solid computer skills; experience with EMR preferred

Responsibilities

• Make home visits for speech, language, and swallowing evaluations
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and family members on rehabilitative care
• Document interventions using electronic medical records (tablet)
