Description

You will provide in-home speech therapy to adult and geriatric patients in the Collegeville area, helping them communicate and participate in daily life. This role offers flexible scheduling, consistent patient volume, and a supportive team environment.

Requirements

• Active Speech Language Pathologist (CCC SLP) license in good standing
• BLS CPR certification
• Reliable transportation for home visits
• Comfort with technology (email, texting, EMR systems)

Responsibilities

• Deliver in-home speech therapy services to adult and geriatric patients
• Evaluate patient communication, cognition, and swallowing to develop treatment goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate home health care documentation per Medicare standards
