Description

You will make home visits to perform evaluations and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. You'll develop treatment plans and educate patients and families, working as part of a multidisciplinary team to help clients remain at home.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational requirements
• Solid computer skills; prior EMR experience preferred
• Availability 1-2 days per week and willingness to travel to multiple counties

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluations and physician orders
• Revise care plans and participate in multidisciplinary care planning
• Educate patients and families on rehabilitative care and activities for independent living
