🎯 What You'll Do

• Serve as attending physician for assigned SNF patients.
• Collaborate with NPs/PAs to manage daily patient care.
• Provide primary care including diagnosing and treating conditions.
• Participate in quality initiatives like Chronic Care Management.

📋 Requirements

• Active U.S. medical license as MD or DO.
• Board-certified or board-eligible in a relevant specialty.
• Ability to perform essential job functions with or without accommodation.
