Requirements

• Active LPN license in Indiana
• Graduation from an accredited nursing program
• Commitment to providing high-quality, compassionate care

Responsibilities

• Provide 1:1 skilled nursing care to clients of all ages in their homes
• Build consistent, meaningful relationships with clients
• Document patient care using electronic charting system
• Follow personalized care plans and collaborate with clinical team
• Maintain flexibility to work various shifts including daytime and overnight
