Description
You will be the first point of contact for prospective participants, guiding them through the enrollment process for our PACE program. Your work will directly drive participant growth and strengthen community engagement, ensuring seniors and their families receive exceptional support from inquiry to enrollment.
Requirements
- Associate's degree in related field (Bachelor's preferred)
- Minimum 1 year experience in healthcare, outreach, or customer service
- Strong communication and organizational skills
- Ability to manage multiple scheduling tasks
- Knowledge of Medi-Cal or healthcare enrollment preferred
Responsibilities
- Serve as primary contact for inquiries, responding to calls and conducting timely follow-ups
- Communicate PACE enrollment process and ensure high conversion from inquiry to in-home visit
- Address financial eligibility concerns including Medi-Cal and Share of Cost issues
- Schedule and facilitate Enrollment Conferences and initial assessments
- Represent Seen Health at community events and collect participant information
