Description

You will provide home health occupational therapy to adult and geriatric patients in Baltimore County. You'll perform evaluations, develop care plans, and educate patients and families to promote independence and safety at home.

Requirements

• Current state Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Perform thorough evaluations of patients' functional status and develop individualized intervention plans.
• Assess home environment for hazards and recommend equipment adaptations.
• Participate in multidisciplinary care planning and revise therapy plans as needed.
• Educate patients and family members on rehabilitative care and safety.
• Accurately document patient care using a touch pad tablet.
