As a Nurse Practitioner at Bold, you will deliver high-quality telehealth care focused on prevention and lifestyle medicine to older adults. You'll collaborate with a clinical team to develop personalized care plans and help shape virtual care protocols.

Requirements

• Active NP licenses in CA, TX, and/or PA
• 5+ years of clinical experience
• 2+ years of telehealth experience with high EMR proficiency
• Strong foundation in preventive and lifestyle-focused medicine
• Excellent communication and empathy-driven clinical approach

Responsibilities

• Deliver preventive and lifestyle telehealth care to older adults (65+)
• Provide feedback on clinical protocols and scalable workflows
• Use EMR for documenting patient information and communicating with providers
• Partner with care coordinators and health coaches to develop personalized care plans
• Manage chronic conditions such as weight, hypertension, and diabetes
