You'll assist clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services, you'll help develop discharge plans. 🎯 What You'll Do

• Assist clients and families with social and financial issues related to continuing care admissions
• Collaborate with Community Care Coordinator, Transition Services on discharge planning
• Communicate with clients and families about progress toward appropriate discharge location
• Coordinate with internal teams to support client transitions

📋 Requirements

• Graduation from a recognized degree program in Social Work or equivalent
• Registered with Alberta College of Social Workers
• Valid Driver's License
• Minimum 2 years experience in clinical and continuing care roles

✨ Nice to Have

• Gerontology experience preferred
