Description

You will provide geriatric psychiatry and behavioral health services in home, assisted living, memory care, and skilled nursing settings. You'll manage your own panel while collaborating with primary care, palliative care, and neurology teams to address dementia-related behaviors, depression, and complex psychiatric comorbidities.

Requirements

• Active PMHNP-BC license
• Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry
• Strong diagnostic and medication management skills
• Ability to practice independently while contributing to a team-based model
• Leadership potential with interest in growing a successful patient panel

Responsibilities

• Comprehensive psychiatric evaluations and medication management
• Management of behavioral symptoms in dementia and cognitive decline
• Collaboration with PCPs, palliative care, and facility partners
• Caregiver education and relationship-based care
• Documentation, care coordination, and panel management
