💼 About This Role

You'll provide direct nursing care to frail elderly participants in the WelbeHealth clinic. Your work ensures seniors receive comprehensive medical care and stay in their homes and communities. This role offers standard business hours and a supportive team environment.

🎯 What You'll Do

• Perform and document ordered wound care in clinic.
• Administer medications and manage medication orders.
• Coordinate medication reconciliation during new participant visits.
• Perform pre and post operative clinical touchpoints and follow-ups.

📋 Requirements

• Graduate of an Accredited School of Nursing with unencumbered LVN/LPN License.
• Minimum one year of nursing experience in a clinical setting with frail or elderly population.
• Must obtain CPR and First Aid certification within 180 days of hire.
