You'll provide home health visits to adult and geriatric clients, performing diagnostic tests and developing individualized care plans. Your work directly improves clients' health, safety, and independent living.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements for mobility and strength
• Develop and implement individualized care plans and interventions
• Educate clients and families on rehabilitative care

📋 Requirements

• Current VA Physical Therapist license
• Graduation from an APTA-approved program
• Ability to work independently and manage time effectively
• Solid computer skills
