Description

As a home health physical therapist with BAYADA, you will provide skilled rehabilitative care to adult and geriatric patients in their homes, helping them regain independence. You will develop and implement individualized care plans, conduct diagnostic tests, and educate patients and families.

Requirements

• Current license as a Physical Therapist in Massachusetts
• Prior home health experience not required; new grads encouraged to apply

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, balance, etc.)
• Develop and implement individualized care plans including manual therapeutic exercises, gait training, and balance interventions
• Assess and revise Physical Therapy care plans, participate in multidisciplinary care plan
• Educate clients, family members, or representatives on rehabilitative care and activities for health, safety, and independent living
• Accurately document observations, interventions, and evaluations using a touch pad tablet
