💼 About This Role

You'll perform home health visits for adult and geriatric patients in senior living communities. You'll conduct thorough evaluations and develop individualized intervention plans to help clients remain at home. This role offers autonomy and the support of a multi-disciplinary team.

🎯 What You'll Do

• Conduct home visits to assess functional status and ADLs
• Develop individualized intervention plans to achieve outcomes
• Evaluate home environment and recommend equipment modifications
• Document patient care using electronic medical records
• Educate patients and families on rehabilitative care

📋 Requirements

• Current VA Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Minimum one year of recent work experience as an OT
