Description

You will provide skilled nursing and rehabilitative care to adult clients in their homes, performing duties such as medication administration, wound care, and monitoring client conditions, enabling them to receive medical care while remaining at home.

Requirements

• Current LPN license in Pennsylvania
• Minimum one year of recent verifiable LPN experience
• Graduation from an accredited nursing program
• Ability to travel to cases
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Making home visits to clients in designated territories
• Administering medication, wound care, treatments, and procedures
• Monitoring clients' conditions and reporting changes to management
• Following and documenting doctors' orders
• Documenting care using electronic medical records on a touch pad tablet
