💼 About This Role

You'll join the largest home health agency in San Francisco, providing hands-on patient care in patients' homes. You'll work under a Case Manager RN to develop foundational nursing skills and support low hospital readmission rates. This entry-level role is designed for RNs aspiring to advance into case management.

🎯 What You'll Do

• Provide hands-on patient care including wound care, tube management, and IV administration
• Accurately document patient care within 24-48 hours consistent with the plan of care
• Assess and educate patients and families regarding diagnosis and care plan
• Participate in weekly case conferences and coordinate care with interdisciplinary team

📋 Requirements

• Current CA Registered Nurse License (RN)
• 1 year of recent nursing experience
• Current Basic Life Support (BLS) card
• Valid CA Driver's License with acceptable driving record
