You'll lead a multi-disciplinary team in providing exceptional client service and clinical excellence in a Medicare-certified home health office. You'll manage field staff, process referrals, and ensure adherence to policies. This role offers flexibility and work-life balance in a supportive environment.

🎯 What You'll Do

• Direct and coordinate care provided by a multi-disciplinary team
• Process incoming referrals and manage case loads
• Mentor and evaluate clinical associates and field staff
• Review OASIS documentation and Home Care Home Base (HCHB)

📋 Requirements

• Active Hawaii Registered Nurse license
• At least 2 years of nursing experience
• Recent Medicare Certified Home Health Care experience
• Comprehensive knowledge of Medicare Conditions of Participation
