💼 About This Role

You'll provide hospice care through comprehensive evaluations and face-to-face assessments to determine hospice eligibility. You'll manage terminal illnesses by developing individualized plans of care and prescribing treatments, collaborating with an interdisciplinary team.

🎯 What You'll Do

• Complete history and physical examinations for hospice patients.
• Prescribe medications and treatments for terminal illness management.
• Perform face-to-face visits for hospice recertification eligibility.
• Participate in interdisciplinary team meetings and coordinate care.

📋 Requirements

• Advanced graduate education and clinical training as a Nurse Practitioner.
• Active state license as a Registered Nurse and Nurse Practitioner.
• Knowledge of hospice regulations and Medicare conditions of participation.
• Ability to work under supervision of a collaborating physician.
