Description

You will provide clinical care and case management for seniors in the PACE model, serving as a communication link among patients, providers, and staff. You will triage patient needs, perform procedures, educate on disease management, and coordinate care plans to help participants age in place with dignity.

Requirements

• Current state-issued Nurse Practitioner or Physician Assistant license
• Bilingual English/Mandarin proficiency
• Strong documentation skills
• Ability to commute to Alhambra
• Influential communication skills for care management conversations

Responsibilities

• Triage inbound communications and prioritize patient needs
• Assist with wound care, IV therapies, and clinical procedures
• Oversee post-appointment processes: diagnostic orders, referrals, medication management
• Develop and revise plan of care based on nursing process
• Educate participants and families on chronic disease, self-care, and prevention
