Description

You will make home visits to adult and geriatric clients in Nassau/Northern Duval County, FL, performing duties such as administering medication, wound care, treatments, monitoring conditions, and documenting care using an electronic medical records system. This role allows you to help clients remain at home with comfort, independence, and dignity.

Requirements

• Current LPN license in Florida
• Minimum 1 year recent experience as an LPN
• Graduation from accredited nursing program
• Ability to travel to assigned cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to clinical manager
• Follow up on and document doctors' orders
• Perform client assessments and case management
