You'll conduct remote assessments for home health patients, act on predictive health signals from Circadia's AI platform, and coach care teams through virtual touchpoints to reduce preventable hospitalizations. This role combines telehealth visits with real-time data-driven interventions to transform home-based care.

🎯 What You'll Do

• Conduct initial and follow-up telehealth assessments on high-risk patients
• Interpret Circadia platform insights to prioritize interventions
• Complete documentation and charts within 24 hours
• Complete 25+ tele visits per day for designated patients

📋 Requirements

• Certified Nurse Practitioner in CA with ANCC in Adult/Family/Geriatric/Acute
• 2+ years NP experience with complex adults in home health or post-acute
• Comfortable with telehealth and virtual care platforms
• Bilingual English and Spanish

✨ Nice to Have

• Tech-savvy with Google Suite, multiple EMRs (HCHB), Slack, AI tools
• Spanish preferred
