Description

You will provide one-on-one physical therapy in patients' homes, helping them recover from injury, illness, surgery, or hospitalization. This role offers flexible scheduling, hands-on training, and support from an interdisciplinary team to reduce burnout and promote work-life balance.

Requirements

• Active and current Physical Therapist license
• One year of clinical experience preferred
• Ability to travel locally to assigned cases
• Willingness to train for home health if not experienced

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and adjust individualized care plans including therapeutic exercises, gait training, and balance
• Educate clients and families on rehabilitative care and independent living activities
