You will provide advanced, hands-on care to infants, children, and adults in their homes, supporting safe transitions from hospital to home care and mentoring ongoing care teams. This role is ideal for experienced nurses who thrive in dynamic environments.

Requirements

• Current license as a Licensed Practical Nurse (LPN)
• Home care trach/vent, NICU, PICU, or ICU experience required
• Passing score on high-tech Adult and Pediatric simulations
• Current CPR certification
• Ability to travel throughout Ocean County

Responsibilities

• Provide direct bedside nursing and personal care for clients with medically complex needs
• Support safe transitions from hospital/rehab teams to ongoing home care
• Deliver expert clinical interventions and ensure continuity of care
• Train and mentor the ongoing care team to maintain quality standards
