Description

As a home care Occupational Therapist with BAYADA, you will provide rehabilitative care to adult and geriatric patients in their homes, helping them achieve independent living through individualized intervention plans and environmental adaptations.

Requirements

• Current Pennsylvania Occupational Therapist license
• Minimum one year of recent work experience as an OT
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of functional status, ADLs, IADLs, and cognitive/motor aspects
• Develop and revise individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and families on rehabilitative care and document using touch pad tablet
