Requirements

• Minimum age 18
• Completed 9th grade, ability to read/write; high school diploma or GED preferred
• CNA/STNA certification on Ohio registry, active
• CPR required
• Minimum 1 year direct patient care experience; valid driver's license and insured transportation

Responsibilities

• Provide personal care with respect and dignity
• Observe, record, and report patient care, symptoms, and emotional/physical status
• Assist in feeding patients as needed
• Attend in-services and interdisciplinary team meetings
• Travel to patients between Lebanon, Middletown, and Springboro
