💼 About This Role

You'll serve as an attending physician and medical director at skilled nursing facilities in Midland, MI, providing patient-centered primary care and overseeing daily care with support from NPs/PAs. Your work will build trust and continuity of care for vulnerable populations, supported by flexible scheduling and intuitive technology like an EHR and AI Scribe.

🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Diagnose and treat acute and chronic conditions, delivering high-quality primary care.
• Participate in Chronic Care Management, Transitional Care, and Remote Patient Monitoring initiatives.

📋 Requirements

• Active medical license (MD/DO) in the United States.
• Board certification or board eligibility in a relevant specialty.
• Ability to perform essential job functions with or without reasonable accommodation.
