Description

You will provide fully remote, video-based primary care including preventive, acute, and chronic care, supported by a clinical AI platform and a dedicated support team. Manage a patient panel, work on population health initiatives, and reduce hospital readmissions.

Requirements

• Board-certified in Family or Internal Medicine
• 3+ years post-residency work experience
• 2+ years in-person primary care experience
• Active OR medical license, IMLC preferred
• Experience with EPIC EMR

Responsibilities

• Provide preventative, acute, and chronic care in a fully-remote virtual setting
• Manage a dedicated panel of patients as their primary care physician
• Refer patients to specialists or acute care when needed
• Work on population health initiatives (AWVs, care gaps, transitions of care)
• Support patients post-discharge to reduce readmissions
