🎯 What You'll Do

• Provide skilled evaluation and treatment in patients' homes
• Address home safety, medication management, and falls prevention
• Serve as educator and patient advocate on interdisciplinary team
• Develop care plans for chronic disease and dementia management

📋 Requirements

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
