💼 About This Role

You'll provide compassionate home health care to adult and geriatric clients in Western Jacksonville, FL. Following a designated care plan and performing assessments, you'll make a direct impact on patients' ability to remain at home with dignity. This role offers flexible per-visit scheduling and the support of a dedicated care team.

🎯 What You'll Do

• Follow a designated care plan per patient's needs
• Make home visits and administer medications, wound care, treatments
• Monitor clients' conditions and report changes
• Document observations and interventions using electronic records

📋 Requirements

• Current RN license in Florida
• Minimum 1 year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Valid Florida Background Screening Clearinghouse clearance
