💼 About This Role

You'll provide home health visits to adult and geriatric clients in Independent and Assisted Living communities in Summit County. You'll develop and implement individualized care plans to promote independent living. This role offers a $5,000 sign-on bonus.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility and strength.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care.

📋 Requirements

• Physical Therapist license in Ohio.
• At least one year of recent work experience as a PT.
• Graduation from a program approved by APTA or CAAHEP.
• Ability to work independently and manage time effectively.

✨ Nice to Have

• Experience with electronic medical records (EMR).
• Strong interpersonal and organizational skills.
• Prior home health experience.
