Description

You will provide skilled nursing and rehabilitative care to adult and senior clients on a per visit basis, enabling them to receive medical care at home. As part of a multi-disciplinary team, you will monitor conditions, perform assessments, coordinate care, and document observations.

Requirements

• One-year prior clinical experience as a Registered Nurse
• Current active Colorado RN nursing license in good standing
• Graduation from an accredited nursing program
• Home Health and OASIS documentation experience preferred
• Willing to complete 4-week orientation if no home health experience
