💼 About This Role

You'll provide skilled home health nursing to seniors in the WelbeHealth PACE program, empowering them to live independently. You'll coordinate care plans with an interdisciplinary team to ensure convenient access to essential health services. This role differs by avoiding OASIS assessments and offering hourly pay that includes travel time.

🎯 What You'll Do

• Conduct home health nursing assessments for equipment needs and participant goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions per care plans.
• Track home health scheduling and coordinate care transitions.

📋 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN License.
• Minimum one year of nursing experience with frail/elderly population.
