💼 About This Role

You'll serve as the primary point of contact for clients with complex needs, coordinating care across providers in a hybrid role. 🎯 What You'll Do

• Assess member needs across physical health, mental health, and social supports
• Develop and oversee client care plans and goal settings
• Conduct outreach and engagement activities for ECM program linkage
• Maintain up-to-date patient health records in EMR and other systems

📋 Requirements

• Active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California
• 1+ year of experience as a care manager or community health worker
• Willing to work Monday-Friday 8:30am-5:00pm, hybrid field and remote
• Clean driving record, valid driver's license, and reliable transportation

✨ Nice to Have

• 2+ years of care management experience
• Bilingual English/Spanish
• Working knowledge of government and community resources
