You will provide home health physical therapy to adult and geriatric patients, helping them maintain independence through personalized care plans, diagnostic tests, and rehabilitative interventions.

Requirements

• Current Physical Therapist license in Massachusetts

Responsibilities

• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Continually assess and revise care plans
• Educate clients and family on rehabilitative care
• Document observations and interventions using a tablet
